Athletic Clearance
You must register by following the below process to participate with PVHS Boys and Girls Swim and Dive Team. Each athlete must be medically cleared/approved, and complete all FHSAA videos prior to the start of voluntary practice (if you plan to participate) or August 12, 2024. ALL CLEARANCE PAPERWORK SHOULD BE SUBMITTED ONLINE BY JULY 31, 2024 to give adequate time for clearance before tryouts.
Please follow the following steps for registration -
1. Mandatory NFHS Courses to be completed by the Student Athlete - https://nfhslearn.com/courses - On the left margin, select "student", "all" and "free". From there, student athletes need to complete the following three (3) courses:
- Concussion in Sports
- Heat Illness Prevention
- Sudden Cardiac Arrest
When you complete each video, be certain to save the certificates to your computer so can you can upload each during the athletic clearance.com process.

All athletes and their parents/guardians must fill out the following at https://www.athleticclearance.com in order to participate with the team. Athletes will not be allowed to tryout on August 13, 2024 until these forms have been completed and verified (NO EXCEPTIONS).
YOU ONLY NEED TO SUBMIT PAGE FOUR OF OF THE EL2!
BOTH PARENT AND STUDENT ATHLETE MUST SIGN/DATE PAGE 4 OF THE EL2!
MAKE SURE THE DOCTOR SIGNS AND DATES AND STAMPS PAGE FOUR!

* To complete this clearance in full, you should have a fully signed and scanned EL2 physical form (page 4) from your doctor and reviewed the courses in bullet #1. All of this will be completed through the https://www.athleticclearance.com or follow the links from our PVHS Athletics page.
